Appointment Cancellation Policy: If the appointment is not cancelled within 24 hours, there will be a$25.00 non- refundable deposit required to reschedule. That deposit will be good for one year and credited towards the next appointment. Anesthetic Procedure Late Cancellation: If an anesthetic procedure is not cancelled within 24 hours, there will be a $75 deposit required to reschedule and will be good for one year and credited towards the rescheduled procedure. *

No-Show Policy: No-Show appointment is when you fail to show up for the scheduled appointment time. Appointment No-Show: If you fail to show foryour appointment, therewill be $58.00 non-refundable deposit required to schedule your next appointment which will be credited towards that appointment. After the 2nd no-show, there will be a $58.00 non-refundable no-show fee in addition to a $58.00 deposit that will be credited toward the next appointment. Surgery No-Shows: If you fail to show for a scheduled anesthetic procedure, there will be a $150.00 deposit required to reschedule and will be credited toward the rescheduled procedure. After the 2nd no-show, there will be a $150.00 no-show fee that will be charged to your account, and we will NOT be rescheduling that procedure. *
